Family Driven Planning Pilot Project!

This project is called Family Driven Planning.  The goal of Family Driven Planning is to help create a supportive network for individuals with developmental disabilities and their caregivers.

The project is for people who:

  • Have a developmental or intellectual disability
  • Currently eligible for or receiving IRA residential services
  • Between the ages of 8 to 35 years
  • Eligible for Medicaid services

Interviews will include 2 surveys on supportive relationships, quality of life, and Medicaid services.  Individuals will receive $5 gift cards for each interview.  There are no significant risks to those who participate.
